Anonymous feedback

Typically, schools collect anonymous feedback from students at the end of each semester. This is too late! I want to address any student comments, concerns, or criticism as soon as possible. To facilitate this, there is a form below for providing anonymous feedback. You may use this form to share anything you would like me to know, either good or bad. This might include what you do or don't like about class, homeworks, or exams, feedback on my teaching style, or topics you may want to learn about. Please be detailed and constructive.
